Do not complete this form if:

You were accepted for undergraduate admission to EMU, but have not yet earned any credit at EMU. Contact the Admissions Office, (734.487.3060), to apply for admission
You attended EMU as a continuing education, 双录取, guest or non-matriculated student. Contact the Admissions Office, (734.487.3060), to apply for admission.
You earned a bachelor’s degree from EMU or another institution and wish to pursue a different bachelor’s degree. Contact the Admissions Office, (734.487.3060), for a second bachelor’s degree application.
You have earned a bachelor’s degree and wish to seek teacher certification only. Contact the Admissions Office, (734.487.3060), to apply for admission.
You were academically dismissed from EMU the last time you attended. Complete the Request for Reinstatement form found on the University Advising and Career Development Center website.

I understand that misrepresentation or omission of any information may be cause for denial or cancellation of re-enrollment OR may result in being suspended or dismissed from Eastern 密歇根 University, if discovered subsequent to re-enrollment. I understand that all pertinent rules and policies of Eastern 密歇根 University that applied to me upon admission are still applicable, as well as new policies and procedures that may have been implemented during my absence, and that it is my responsibility to become knowledgeable about them. I understand that all credentials submitted in support of my re-enrollment become property of the University and are not returnable.

Processing takes approximately one week. Students will be notified by mail at the address provided above about when and how to register after their re-enrollment has been processed, or they will receive alternative correspondence explaining additional steps needed for re-enrollment.
